In a bunker hidden beneath a Russian forest, Samyaza Initiative scientists conduct secret bio research to create genetic hybrids with DNA from humans and an alien specimen dubbed Subject M. A creature gets loose, killing everyone in the facility and infecting them with a virus that reanimates dead humans as mutant monsters. Chloe, the sole survivor, broadcasts a distress message pleading for her rescue as well as recovery of the research.Despite his pregnant wife Jessica's objections, James Beckett accepts a Special Ops mission to rescue Chloe with his friend and fellow mercenary Junior Marshall. While Marshall's ex-girlfriend Rachel joins Alpha Team outside one end of the facility, Beckett and Marshall join Moses and Preacher of Bravo Team at the other end. Both units are instructed to extract Chloe without attracting attention from Olek Volkov, who leads a separate assault on the bunker with a hostile Russian militia and his henchman Jonah Petrov.Upon breaching the bunker, Alpha Team encounters a mutated human. Rachel is severely wounded. Her companions are killed.Bravo Team engages Olek's men in the forest. Marshall kills Jonah's brother Jacob. Moses dies. Olek and Jonah capture Preacher. Beckett and Marshall escape the firefight by jumping off a cliff into water.Beckett and Marshall make it inside the underground lab. Beckett likens the lab's research to the biblical legend of the Nephilim after computer files reveal that the scientists were breeding beings using alien DNA.Beckett and Marshall find Chloe and fend off the creature. Chloe recounts how the lab's scientists originally extracted Subject M's blood to create a genetic hybrid named Adam 30 years ago. Chloe claims Olek was in charge of guarding Adam, but he supposedly killed Adam and set the creature loose in an attempt to steal the research for himself.One of Olek's men tries to stop Beckett, Marshall, and Chloe at gunpoint. The creature kills him.Beckett, Marshall, and Chloe find Rachel wounded. Recognizing that she is about to transform into a monster, Chloe kills Rachel, which greatly upsets Marshall.The creature attacks again. Beckett and Chloe barricade themselves in an infirmary. While Marshall hides in the morgue, an undead body bites him before he can stab it in the head.Jonah takes revenge for his brother's death by capturing Marshall. While restrained to a table, Marshall's infected bite wound causes him to see a hive mind flashback. The vision shows Marshall that Chloe was actually the genetic hybrid created in captivity 30 years ago, and she released the creature in an effort to escape the facility while Olek tried to stop her. After Marshall learns the truth, the creature kills Jonah.Beckett realizes Chloe is pregnant. Chloe confesses she is Adam and admits she orchestrated the creature's breakout so she could kill her captors and escape with her unborn child.The creature captures Beckett. Having escaped his restraints, Marshall enters and forces the monster to release Beckett by threatening to shoot Chloe. Beckett retrieves a flamethrower that he uses to torch the creature to death.Chloe escapes outside where she runs into Olek. Claiming she never meant for him to get hurt, Chloe apologizes to Beckett, but then pulls a knife and stabs him. Olek stabs Chloe in retaliation. They both fall to the forest floor.Beckett and Marshall make it outside to find Chloe on the ground. Chloe suddenly attacks both men with her knife. A physical fight ensues until Beckett and Marshall kill Chloe.Marshall continues mutating from his bite wound. Beckett leaves Marshall behind to collapse.Beckett calls his wife. Before he can say a word however, Beckett collapses from the wounds suffered in his fight with Chloe. When an extraction team arrives later, Beckett mutates into a monster and attacks.
